Thank you for taking part in NCCPE Seminars. This form will collect the permissions needed for us to share your contribution with our audiences.

As part of this form, we will ask for details that can be used to identify you –  e.g. your name, email address, job title, institution, location, etc. This information is needed so that we can process your permission form and use your contribution for the purposes outlined in this form. Contact information such as Email Address, Address or Telephone number will not be published in any publications or the website or other digital assets. 

We will store all your responses securely in files held on Microsoft services hosted by the University of the West of England, Bristol. The data are accessible only to collaborators identified by the project or service leader and access requires multifactor authorisation using institutional names and passwords. Your responses will be stored for 7 years from the date of submission, after which time they will be permanently destroyed.
